Below are my office policies for your convenience. There are two documents, one is titled Office Policies and the other is the Intake Form. Please read through everything and sign where indicated. It works better to bring the paperwork with you to the appointment, as I do not have a secure server for emailing it back. Please do not email or submit electronically due to HIPPA requirement.
